Baptism Booking Form for a Child

There is some preparation to go through before a baptism can be arranged, so please make contact in plenty of time.

Please specify all Christian names (first names) for the child, including any middle names
Please give the full postal address for the child (where they live) at the time of their baptism. Please enter the full address, as this website does not have the facility to look this up using your postcode.

Please specify how you would like the child's father's occupation to be recorded. If you no longer work outside the home, you can either specify this (using "father" or "house-husband"), or you can indicate your previous occupation.

Please specify how you would like the child's mother's occupation to be recorded. If you no longer work outside the home, you can either specify this (using "mother" or "housewife"), or you can indicate your previous occupation.

Godparents
It is customary to have 3 godparents. When baptising a boy, you would usually choose 2 men and one woman; for a girl, you would usually choose 2 women and one man. This is, however, only a custom, and you can have more godparents if you wish. This form gives you space for up to 5, which is the most that there would normally be.
